However, you may enroll in the on-line version and have access to practice exams, class video, links, resources, etc...
Dr. Nicholas Cooper-Lewter will provide the
training based on exam preparation guides created by the Association for Advanced Training in the Behavioral Sciences. The College is making arrangements with the publisher to procure the training manuals at a reduced rate for our students. It is not necessary to purchase the AATBS manuals to attend the training. Late Add (AS-199) Form

If a student does not commit his/her financial aid or does not pay his/her fees, their classes will be dropped. This form will need to be signed by the professor and the student’s Dean not the course’s Dean. If a student accidentally drops the wrong course, this form will be used to re-instate the student into the class. If the student has to change sections after the last day to add classes, this form will need to be signed by the professor of the new section and the student’s Dean not the course’s Dean.

This form is completed when the student completes an "incomplete" and the instructor/student is seeking a removal of the incomplete status.

An Assignment of Incomplete Grade form is completed by the instructor in VIP as part of the usual grade submission process. The justification for the incomplete grade, conditions for make-up, a deadline for completion, and a back-up grade if the course work is not completed by the deadline must be included on the form. Re-enrolling in a course will not make up an incomplete grade. A grade of I is not computed in the calculation of a student’s cumulative grade point average until the make-up grade is posted.

If the professor believes there is academic justification for an extension beyond the one-year limit, a request for extension should be submitted to the dean of The Graduate School before the expiration of the year, specifying the justification and specific duration of extension requested (GS-47 form Extension of Incomplete Time Period Authorization).

Grade changes based on transcription or computation errors shall be reported directly to the Office of the University Registrar on the appropriate grade change form signed by the instructor and the head of the student's academic unit.

Any request for a grade change must be submitted by the instructor no later than one calendar year from the date on which the grade was reported. Beyond this period, grade changes will be considered only in exceptional circumstances and must be handled through the petition procedure of the student's college. Any other grade change request resulting from enrollment discrepancies, medical withdrawals, or perceived administrative errors (changes to W, WF, audit, credit, S/U, or to I) must be submitted on the appropriate forms with signatures and documentation to the dean of the student's college for review through the petition procedure.

The purpose of an independent study is to enable a student to gain an in-depth knowledge of an area germane to the student's course of study which is not offered in a course in the School. A student may not audit or sit in on a course and later receive independent study credit for the course or material covered in the course.

Complete this form if you are exceeding the following limits (click the "read more" button) regarding the number of enrolled credit hours:
SUMMER TERMS: A two course load (6 semester hours) is considered maximum for eitherthe Summer I or Summer II term in accordance with the CSGS policy.The exception to this policy which remains within the CSGS policy is for enrollment in athird course which lists a database session ending before the other two course enrollmentsbegin (i.e., a course assigned to the Summer I (May Session) term, or a third course which listsa database session extending over virtually the entire summer (or longer). In other words, thethird course cannot generally duplicate the Summer I or Summer II term.
FALL/SPRING TERMS: In a fall or spring semester, a 15 (fifteen) credit hour maximumcourse load is consistent with the CSGS policy. Some academic units have set a 12 (twelve)credit hour maximum. Any higher enrollment requires the recommendation of the GraduateDirector or Chair of the unit for The Graduate School to authorize the maximum 15 (fifteen)credit hour load for students in those programs.
