There are three methods to complete the forms. First is the electronic submission of forms. These forms will prompt you to print a copy before submission. These forms are in the left column.

There are forms that you can complete using Microsoft Word that can be printed and sent back to the Field Office. These forms are listed in the right column of this webpage.

The third method is to print the form out using Adobe Acrobat from the list on the middle column. You will need Adobe Acrobat to use these forms. For a free version of Acrobat Reader, click on the icon below.

The Microsoft Word and Adobe Acrobat forms are identical, you should choose one of these methods, not both.
